You will need to print out the application of the scholarship category for which you wish to apply (see color-coded list below). You may apply for ONE scholarship category ONLY. Brown - Business and Communications—Finance, Economics, Marketing, etc. Yellow - Community Service/Social Services —helping professions Red - General Academic—and other majors Application Due Date Applications must be postmarked by midnight, April 21, 2017, to qualify. All materials required must be submitted by that date/time in order to qualify the application for consideration (even if the materials are coming from various sources, e.g., the applicant, a school registrar’s office—in the case of transcripts— letters of recommendation, etc.). The Application Form Please fill out all sections of the application—both pages. Official transcript of grades—in an envelope sealed and stamped by the registrar’s office for the final three reporting periods at the high school and/or college where you last enrolled. A one-page essay—Include your essay, which addresses the topic listed on the front of your application. A brief summary—Include your summary of how you meet the judging criteria, future plans, community service and special skills. A certified copy of your U. S. Birth Certificate or U. S. Naturalization Certificate must be included. Two letters of recommendation from a school counselor, business leader, teacher or religious leader are required. You must include the provider's printed name, title, and telephone number. Sign the application. If you are younger than 18 years of age, your parent or legal guardian must also sign the application. The application must be complete and must be postmarked by April 21, 2017.
